I declare that by joining the FM GROUP Club, I express my willingness to participate in it and agree to the conditions set out in the Marketing Plan, and in the FM GROUP Club Rules.I have received a copy of the Marketing Plan, and the FM GROUP Club Rules, and I am familiar with their contents and accept them in full.

I undertake, during the period of participation in the FM GROUP Club, to comply with the provisions of the FM GROUP Club Regulations, Marketing Plan and Code of Ethics.

I agree to receive commercial information via e-mail or telephone number provided by me, i.e. to receive advertising and notifications related to marketing of products and services that appear marketed under the FM GROUP Brand and FM GROUP Club, under the terms of article 8 of Decree-Law 24/2014, of February 14.

I also give my express and irrevocable consent to FM GROUP World to transfer my personal data to FM GROUP Subsidiaries if this proves necessary for the purposes of the processing identified in this Agreement. FM GROUP Portugal administers the database containing the personal data of FM GROUP Portugal's Business Partners. FM GROUP World is the administrator of the database in which the personal data of all registered Business Patners is stored. Personal data is collected and processed for the purposes of: registeringwith the FM GROUP Club to make purchases and organize the Network, making financial calculations and documenting commercial transactions, calculating points, discounts and commissions, maintaining contact with other Business Partners and their Groups, promoting the most active Business Partners, collaborating with the aim of offering Business Partners products and services in which FM GROUP Portugal or the Affiliate participates, respectively, as well as implementing loyalty and motivation programs.

The recipients of Business Partners' personal data may be: (a) other Business Partners, (b) entities personally or financially linked to FM GROUP Portugal or FM GROUP World, (c) potential customers of FM GROUP Portugal, FM GROUP World or a Subsidiary and third parties who declare their willingness to join the FM GROUP Club, (d) transport companies, couriers, couriers working with FM GROUP Portugal, FM GROUP Subsidiaries and the Business Partners, (e) companies organizing events for the Business Partners at the request of FM GROUP Portugal or FM GROUP World, (f) training companies, (g) advertising agencies, (h) public relations companies and (i) operators.

By completing this application and entering into the Agreement, the Business Partner freely, voluntarily and unconditionally provides his/her personal data to FM GROUP Portugal, FM GROUP World and/or an FM GROUP Subsidiary.

The Business Partner has the right, free of charge and free of any restriction, to verify their personal data and request its deletion or rectification, under the terms of the applicable legislation.

The lack of consent for the processing of personal data, in accordance with the aforementioned provisions, is equivalent to the lack of consent to enter into this Agreement.

I consent to the disclosure by FM GROUP Portugal or the Affiliate of my personal data on the website or mobile applications of FM GROUP Portugal so that they are available to other users of the Network. In case of lack of consent, my data will appear only with the Partner Number (Distributor Number).

I declare that, having the status of Recipient, I will purchase FM GROUP Products for my own use, as well as for members of my family or friends, and that I will not make direct sales of FM GROUP Products purchased or provide advertising services for the Network and FM GROUP Products; I will be entitled to the Commercial Discount.

As an Individual Seller and a natural person, I will sell the products directly and provide advertising services for the Network and the Products. I will not be subject to value added tax (VAT) until I exceed the value of 10,000 EUR in the fiscal year. For providing advertising services for the FM GROUP Network and Products I will be entitled to a Remuneration.

The Company and I will carry out a commercial activity of direct sale of Products and provision of advertising services for the FM GROUP Network and Products, and that I will be subject to value added tax (VAT), for providing advertising services for the FM GROUP Network and Products I will be entitled to remuneration

I give my consent to receive correspondence via e-mail to the e-mail address I have indicated or via an individual panel available on the website www.pt.fmworld.com after logging in.

I give my consent to publish my data and image on the website or in the publications (disclosures) of FM GROUP Portugal, FM GROUP World or the Branch.

FM GROUP Portugal has notified the National Data Protection Commission of the terms and conditions of the processing of Business Partner data.
